Dear Parent,
Welcome to Our Lady of the Mount Office of Catechesis. Below you find the information you need to register your child(ren) in our program. Our Registration Forms are all online now. Please click on the appropriate form. You can then print it out and fill it in by hand or you can type the information into the form and then print it out. Return all forms to the office with your payment. Please review The Parent Handbook before completing the registration forms. The booklet outlines our polices and procedures for the program.
Explanation of Forms
Families who are re-registering, after reading the Parent Handbook, please complete and return the following forms: Parent Agreement, Re-registration, Food Allergy/Special Needs, Parent Participation and Honeywell Instant Alert (for new students only).
New families (those families who currently do not have a child registered in the program) please contact the office to register at 908-647-1402. Please confirm you are registered in the parish.
Parent Handbook - Please read the Parent Handbook prior to completing all forms. This handbook contains polices and procedures for our Religious Education Program.
Parent Agreement Form - The parent or legal guardian has read, understood, and agreed to comply with all the policies and procedures listed in the Religious Education Parent Handbook.
Re-Registration Form - This form to be completed by returning families. Returning families with a first grader should add the new student to this form and complete the "New Registration" form. (Late fee does not apply to new registrants.)
New Registration Form - Complete for all new students, in addition, please submit a copy of each new student's baptismal certificate. Families must also complete the “Re-registration” form (with emergency contact information) with the “New Registration” form and return it with their packet.
Food allergy/special needs Form - Information will be shared with teacher and kept confidential.
Parent Participation Form - Our program relies on volunteers, please consider assisting.
Bus Permission Slip Form - Hold until September and submit to your child's elementary school teacher.
Honeywell Instant Alert Form - Complete and return. Honeywell is used for inclement weather cancellations, as well as disseminating program information. Complete and return. We require all families to register with the Honeywell Instant Alert system. Please note: Our program is independent of the local elementary schools. Separate registration is required for both. Registration for the system will not be available until after August 30. After that date, please go to https://instantalert.honeywell.com to register and view the Honeywell registration instructions.
Special note: All families participating in our program must be registered in the parish and are expected to be attending Mass regularly.
